PRE-SITE CONTRACT

( ) I understand that I am expected to conducted myself in an appropriate manner, be punctual, manage my time, and be a collaborative and cooperative worker.
( ) I will adhere to any sign in/sign out procedures.
( ) I have confirmed transportation plan.
( ) I will adhere to the vacation schedule of the site, not the school.
( ) I know how to notify my site if I am sick, and will do so.
( ) I will incorporate my research, journal entries, & internship experience into my presentation.
( ) I will see my internship through to completion, and change internships only if my mentor and the program coordinators have agreed upon the change.
