HEALTH OFFICE INFORMATION

Incoming 7th Grade Students 2017/2018 School Year

On October 26, 2015, Governor Cuomo signed into law that all 7th grade students MUST receive the Meningococcal Vaccine.   Students will need this vaccine in order to begin 7th grade in September, 2017.  Please contact your health care provider for an appointment and for further details.  Once your child has received this vaccine, please be sure to provide the Nurse’s Office with a copy of the immunization record.  In addition, as per NYS Education Law, “All students entering 7th grade for the 2017-2018 school year must submit an updated physical to the Nurse’s Office”.  A physical exam dated on or after September 5, 2016, would be considered acceptable.

Fall 2017-2018 Information

Sports sign-ups are completed by parents online during the summer.  Please check the district website for more sports information.  Any students who would like to play a fall sport will need a current physical exam, dated on or after September 5, 2016, to include blood pressure, hearing and vision testing.  The exam must also state “Student may participate in Sports Program”.
