HEALTH OFFICE INFORMATION
New York State mandates that all 6th, 7th, and 8th grade students attending public school be screened for scoliosis. Scoliosis is the abnormal curve of the spine, and if left untreated, can cause future health problems. If your child has had a physical exam with their health care provider, and if “Scoliosis” is marked on the exam, this will be acceptable.
Sign up for spring sports for the Lakeland Central School District will begin in February for high school athletes and in March for middle school athletes. If you would like to play a sport for school, then a current physical exam is required to be on file in the Nurse’s Office. Keep in mind that a physical exam is valid for one year from the date noted on the exam. Vision and hearing results are mandated requirements for sports participation. Should your child need screening, please have him/her check with the Nurse’s Office. Sports sign-ups are completed on line. A student trying out for a high school sport must sign up on their high school’s website. High school sports sign-ups open on Friday, February 24, 2017, and close on Monday, March 6, 2017, at 3:00 p.m. A link to the Chalk Sports Form can be accessed on the high school’s website.
Any LCBMS student who would like to try out for a spring sport can access the Chalk Form on the LCBMS website starting on Friday, March 6, 2017, and it will close on Thursday, March 16, 2017 at 3:00 p.m. All Chalk Forms must be completed by a parent only. Keep in mind, failure to complete the Chalk Form may result in an athlete not being able to play their sport.
If your child requires medication, please ensure that the medication orders are completed and on file with the nurse’s office, since this may jeopardize your student from obtaining a sports card. All information is obtainable on the Lakeland Central School District’s website.
Incoming 7th Grade Students 2017/2018 School Year
On October 26, 2015, Governor Cuomo signed into law Chapter 401 of the laws of 2015 which amended New York State (NYS) Public Health Law (PHL) section 2164 to require children entering or attending Seventh and Twelfth grade, on or after September 1, 2016, to receive an adequate dose or doses of vaccine against Meningococcal Disease, as recommended by the Advisory Committee on Immunization Practices (ACIP). Students will need this vaccine in order to begin 7th grade in September, 2017. Please contact your health care provider for an appointment and for further details. Once your child has received this vaccine, please be sure to provide the Nurse’s Office with a copy of the immunization record. In addition, as per NYS Education Law, “All students entering 7th grade for the 2017-2018 school year must submit an updated physical to the Nurse’s Office”.
