Frostbite and Hypothermia

As a reminder, please alert the health office during the school year to discuss with the school nurse these medical issues:

  • A student has or has recently recovered from a serious illness
  • A student has been seen in the ER or hospitalized
  • A student has had surgery
  • A student has sustained an injury requiring medical treatment

Documentation from your health care provider will be needed to return to school with or without restrictions for Physical Education and recess.  If your child requires crutches in school a MD note is needed.

Physicals are required for all new entrants and students in grades Kindergarten, 2nd and 4th Grade. Documentation of a physical or date of upcoming physical must be in the Health Office by 10/15/14. The School Medical Director will examine students who do not have a physical on file or documentation of an upcoming appointment.  You will be informed of the date of the School Health appraisal. Please send completed physical forms to the Health Office. Please keep a copy for your records.

Any kind of medication cannot legally be dispensed to any child in school without a Health Care Provider’s written order and parental consent.

Physical and Medication forms can be down loaded from the District’s web site click on Departments look for Health Office.

Blue Health Emergency Forms

Thank you for returning your forms to the Health Office. Please update any changes ASAP.  It is important to have numbers where I can reach you.

If your child shows any symptoms of illness: temperature over 100?, flu-like symptoms, vomiting or GI issues, sore throat that interferes with swallowing, ear pain, colored eye drainage, suspicious or draining rash. Please keep him/her home. A child who is sick will not be able to perform well in school. Notify your health care provider if there is no improvement.

Children may not return to school unless:

  • They are fever free for 24 hours WITHOUT medication for fever
  • They have not vomited for 24 hours
  • A FULL 24 hours has passed since their first dose of antibiotic for strep throat or a skin infection.

Absences must be called in daily before 8:45AM.  Please call BFES 245-7444 ext 206 and leave a clear message including name of child, grade, and reason for absence.  Please leave details of illness:   It is recommended that students stay home until at least 24 hours after they are free of fever (100 F or greater) or signs of a fever, without the use of fever reducing medication.

Vision and Hearing screenings are performed during the school year.  All New Entrants and students in Grade 1, 3 and 5th will have a vision and hearing screening. Students in Grade 2 will have a vision screening; Parent/Guardians will be informed of any unusual results.  Any concerns about your child’s vision or hearing please contact the health office.

Please see the district’s web site regarding a HEALTH NOTICE with Information about the Enterovirus EV-D68.
