It is the policy of the Lakeland Board of Education that, in order to participate in sports, student/athletes must possess a valid Authorization for Sport Participation (Sports Card).  Student/athletes who do not have a valid Sports Card are not permitted to “try out for” or “participate in” any interscholastic sports activity/program.

In order to obtain a Sports Card, each student/athlete must have the necessary forms completed.  All three forms must either be on file in the Health Office or brought in on the specified date.  Forms completed by students and signed by parents will not be accepted.

Athletic Health History

Student/athletes are required to have a completed Athletic Health History form on file in the nurse’s office.  Students may obtain this form from the nurse or the main office at WPHS, LHS, or LCBMS or it may be downloaded from the Lakeland website at www.lakelandschools.org.  Click on Athletics, then Forms.  The parent/guardian must complete and sign the form in ink, if it has not been previously submitted.  This form need only be submitted once in an athlete’s high school career.

Annual Physical Examination

Student/athletes must have a valid Annual Physical Examination form dated within twelve months prior to the start of the season.  However, if the student has had a physical in the month the season starts, it will be acceptable until the end of the season.  This form MUST be obtained from the nurse or main office at WPHS, LHS, LCBMS, but a sample can be viewed on the District website.  This form must be completed and signed by a physician.

Please note:  Physical examinations will not be done in school during the summer.

Authorization for Sport Participation (Sports Card)

The Authorization for Sport Participation (Sport Card) form should be filled out by a parent/guardian in inkThis form MUST be obtained from the nurse or main office at WPHS, LHS, LCBMS, but a sample can be viewed on the District website. The completed and signed form should be brought to the school nurse on the scheduled date and time.  The student/athlete must be present to be issued his/her Sports Card.  The student’s vision and hearing will be checked on these days.  If a student wears glasses or contacts, he/she should have them that day. 

Interval Health History

An interval Health History for each student athlete must be completed in ink and signed by a parent/guardian, and submitted to the nurse no more than 30 days prior to tryouts for each season.  This is part of the sport card form.

FALL ATHLETIC SEASON

The Authorization for Sport Participation (Sport Card) form and the Fall Sports Certification Schedule will be mailed home by the coach during the summer to those students who have already signed up for fall sports. Student/athletes will only be certified according to this schedule, which can also be viewed on the District website.  Any student who is not available during their scheduled day and time should bring the necessary forms to the Health Office on the first day of school in September, to be processed by the nurse as soon as possible.

WINTER and SPRING ATHLETIC SEASONS

Approximately a month before the beginning of the season, the school nurse will conduct sports sign-ups during physical education classes, and/or will make an announcement for students to see the nurse for sports sign-ups.  Authorization for Sport Participation (Sport Card) forms will be distributed to those students with interest in participating in a sport.  All forms must be returned to the nurse no later than the Wednesday prior to the season starting date.
